Pt may have decreased discomfort as evidence by pt rating discomfort a 3 or fewer on a level of 0-10, pt capable of change situation and push without and guarding from the abdomen and facial grimacing by the end of the clinical working day. Nursing Interventions
1 . Get pts assessment of soreness to include position, characteristics, starting point, duration, rate of recurrence, quality, depth. Assess soreness on a 0-10 pain range every hour with rolling. Assess pts description of pain. Acknowledge the pain experience and convey acknowledgement of pts response to soreness.
2 . Evaluate vital signs noting heart beat and heart rate q4h. Keep an eye on skin color and temperature, which can be usually improved by severe pain.
several. Asses pertaining to referred discomfort, as ideal.
4. Provide PRN pain medication as per MD order (Hydrocodone/Tylenol 7. 5/325 magnesium PO 3x daily). Evaluate pain on the 0-10 discomfort scale, give medication if pain is 6 or above.
your five. Assess pts acceptable standard of pain and pain control goals. Educate pt to report pain before this gets severe (8/9 on a scale of 0-10).
6th. Provide comfort measures (ie. Touch, transfering, use of hot or frosty packs, nurse's presence, peaceful environment, and calm actions.
7. Examine and file pts respond to analgesia and assist in transitioning or transforming drug regimen, based on specific requirements and protocols....
